The Routes Across State Lines
“Compact” is not one thing. These agreements share a name and almost nothing else — different eligibility rules, different governing bodies, different fee schedules, and different ways to fail. Which one applies to you is decided by your license type, not your preference.
Interstate Medical Licensure Compact (IMLC)
Physicians — MD and DO
An expedited pathway rather than a single multistate license: you qualify once through a state of principal license, then receive a full license from each participating state you select. Eligibility is strict — board certification, no disciplinary history, and a qualifying connection to your principal state.
Nurse Licensure Compact (NLC)
RNs and LPN/LVNs
A genuine multistate license issued by your single primary state of residence, valid in every other member state. Residency is what governs it, not employment, so a move across state lines changes your obligations — and that is where most compliance problems start.
PSYPACT®
Doctoral-level psychologists
Authorization to deliver telepsychology into participating states, built on the ASPPB E.Passport credential. It covers telepsychology from your declared home state — not permanent in-person practice, and not master's-level clinicians.
Counseling Compact
Professional counselors — LPC, LMHC, LPCC
A privilege to practice in each member state, tied to the license in the state where you live. Forty jurisdictions have joined, but only ten were issuing privileges in October 2026, and the counselor must submit the application personally. Our Counseling team handles readiness, exam prep and guided filing, and the full licenses everywhere the Compact does not reach.
ASLP Compact
Speech-language pathologists and audiologists
A privilege to practice in each member state, tied to your home-state license. Thirty-six states have enacted it, but only four are issuing privileges, and only when both your home state and the state you want are among them. You file each privilege yourself; our ASLP team gets you ready and files the state licenses everywhere else.
Dietitian Licensure Compact
Dietitians
Nineteen states have enacted it, but no state issues privileges yet, and the first are expected sometime in 2027. Until then, practicing in another state takes that state's own credential, which our Dietitian team prepares.
Dentist and Dental Hygienist Compact
Dentists and dental hygienists
Enacted in thirteen states, but no state issues privileges and no launch date is set. Every additional state is a full license for now, and our DDS team files licenses by credentials, full applications, and sedation and hygienist permits.
No active compact
Optometrists, podiatrists, behavior analysts, chiropractors and MFTs
There is no optometry compact, and the podiatric compact is not active, so every new state is a full license for optometrists and podiatrists. Behavior analysts, chiropractors and marriage and family therapists have no compact either. Each has its own team that files the state applications.
State licensure by endorsement
Everyone, everywhere a compact does not reach
The route that still does most of the work. Where your target state is not a compact member, or the compact's eligibility rules exclude you, a straightforward endorsement application to that state's board is often both faster and cheaper than forcing a compact route.
Who Handles Your License
Pick your profession. Compacts are separate agreements with separate rules and separate governing bodies, so each one has its own team — and its own site.

Not sure, or your situation spans two of these? Use the form below — it asks the same two questions and routes you automatically, and the lead reaches us either way.
Organization Licensing and Network Denials
Some licenses belong to the business, not the clinician, and a payer network denial is its own process. Each of these has its own team and its own site, and the form below routes you there when you pick the service.
Pharmacy licensing is handled by our pharmacy team
Nonresident pharmacy licenses in every state you ship to, and the in-state licenses your pharmacist-in-charge now needs, for mail-order, specialty and telehealth pharmacies. Tell us your states in the message below and we will scope it.

Agency licensing is handled by our HHA team
Ownership changes, revalidations, state licenses and Medicare filings for home health agencies, hospices and home care agencies. CMS is not enrolling new agencies or hospices while its moratoria last, so we prepare those filings to go in when it reopens.

Facility licensing is handled by our behavioral health team
State licenses, accreditation readiness and opioid treatment program approvals for outpatient, residential and addiction treatment programs. Tell us the program and the state in the message below and we will scope it.

DME supplier enrollment is handled by our DME team
Medicare DMEPOS enrollment, accreditation readiness and every state license you need, in the order CMS requires. Tell us your locations and the states you ship to in the message below.

CLIA certificates are handled by our CLIA team
Certificates of Waiver, PPM certificates, state lab registrations and every change after that, for practices, pharmacies and clinics. Waived tests are not exempt from CLIA.

Network denials are handled by our Network Access team
When a health plan rejects your application or says its panel is closed, that team finds out why and prepares the reconsideration request, the evidence and the letters. Tell us the payer and the state in the message below.

What We Will Tell You Honestly
The counseling and social work compacts are at earlier stages than their announcements suggest. The Counseling Compact has forty members but was issuing privileges in ten as of October 2026, and the Social Work Compact has not issued a license to anyone yet. Our Counseling team tells counselors which of their states can be a privilege and files full licenses for the rest, and our Social Work team files the state licenses social workers need until their Compact opens. Marriage and family therapists have no compact at all; our MFT team files their state applications and Access MFTs endorsements.
The newer compacts are further back still. Thirty-six states have enacted the ASLP Compact, but only four are issuing privileges, and the licensee files each one. The Dietitian Licensure Compact and the Dentist and Dental Hygienist Compact have been enacted but issue no privileges yet. Optometry has no compact, and the podiatric compact is not active, so optometrists and podiatrists need a full license in every new state.
The same applies to eligibility. Compact routes carry conditions — residency, board certification, a clean disciplinary record — and if you do not meet them we would rather establish that on a scoping call than after a non-refundable application fee.
